The Request for Alteration Form can be downloaded from the Manor Creek Farms website, under the E-Forms Tab. Please mail the completed form located on the bottom of the form. To quicken the process, please provide all information such as colors, size, location and a phone number if there are question. Please allow 30 days for review and approval.
